Custom modifications are structural elements added to or removed from specific areas of the insole. They may be intended to change pressure distribution, support certain areas, restrict or free movement, and improve how the foot fits inside the shoe.

Eleven modifications for different areas

In the slides, each modification is presented with its mechanical function and possible clinical applications. Selection should be based on assessment.

Morton’s Extension

Stiffens the first MTP joint and reduces dorsiflexion force at the first MTP and interphalangeal joints. Applications listed in the slides include hallux rigidus, pathology of the second metatarsal head, DJD, and arthritis or injury of the first MTP joint.

1ST MTP

1st Ray Cut Out

Offloads the first metatarsal head and increases joint motion of the first ray. Listed applications include hallux valgus, hallux limitus, a plantarflexed first ray, and sesamoiditis.

1ST RAY

5th Ray Cut Out

Offloads the fifth metatarsal head and increases movement of the fifth ray. Listed applications include Tailor’s bunion, deformities of the fifth metatarsal head, and callus formation.

5TH RAY

Toe Crest Pad

Supports mallet, claw, and hammer toe deformities, relieves pressure at the tips of the toes, and may help separate the toes. Listed applications include toe deformities, calluses, and metatarsalgia.

TOES

Heel Spur Pad

Offloads the plantar tubercle of the calcaneus, distributes pressure, and adds shock absorption and cushioning. Listed applications include heel spur and plantar fasciitis.

HEEL

Dancer’s Pad

Offloads the sesamoids, relieves the first metatarsal head, and cushions the second through fifth metatarsal heads. Listed applications include a rigid plantarflexed first ray, sesamoiditis, and a painful bipartite sesamoid.

SESAMOID

Metatarsal Bar

Elevates the second, third, and fourth metatarsals to shift pressure from the heads toward the shafts. Listed applications include metatarsalgia, neuroma, forefoot callusing, and intermetatarsophalangeal bursitis.

MET BAR

Metatarsal Pad

Distributes pressure across the metatarsal heads and elevates the surrounding heads. Listed applications include metatarsalgia, prominent bone or soft tissue, hyperkeratosis, and foot ulceration.

MET PAD

Metatarsal Relief

Accommodates and relieves the metatarsal head area. Listed applications include plantar corns, callusing or warts, prominent tissue, hyperkeratosis, and a dropped metatarsal head.

RELIEF

Medial Flange

Creates an anti-pronation effect and medial stabilization for the first ray and calcaneus. Listed applications include marked overpronation, severe pes planus, talonavicular joint collapse, and posterior tibial tendonitis.

MEDIAL

Lateral Flange

Supports lateral ankle instability and increases frame stiffness and strength. Listed applications include midfoot supination, stabilization of the fifth ray or calcaneus, and calcaneocuboid joint collapse.

DIGITAL MANUFACTURING PROCESS

Digital modeling and milling of a functional insole

The CAD/CAM process shown in the PLS slides combines foot-data capture, specialized digital design, and precise manufacturing.

Data capture

Foot shape can be captured with a 3D scan, digital scan, foam impression, or physical mold.

CAD modeling

The scanned shape is imported into a CAD software environment designed for orthotic insole design.

Precision milling

The digital model is transferred to the CAD/CAM system to manufacture a custom functional insole with high precision.

File storage

Digital files can be stored in OBJ or STL format for future reproduction or modifications.
